March 2014-AAOS New Orleans-Biologics Meeting Report

The initial and early review of the poster session at the Annual Meeting of the American Academy of Orthopaedic Surgeons (AAOS) 2014, revealed a paucity of biologic/regeneration posters, however, I was however a bit early.  Most posters were in animal models, unfortunately no translational clinical papers in humans, and only a few papers looking at human ex vivo and in vitro work.  The best poster that I saw related to biologics that I saw this morning, was from The Center for Stem Cell and Arthritis Research-Korea.  Lead author is Yun-Jin Choi, M.D., however I believe that this work is from the lab Yong-Gon Koh.  I first met Professor Koh at the ICRS Focus Meeting in Bologna, where his lab was presenting work on Allogenic MSCs used for knee articular cartilage regeneration/restoration.

This paper outlined the Age and Cartilage related differences of synovium tissue derived Mesenchymal Stem Cells (MSCs).

Methods:   3 groups

Group A:  young patients < 30 years old underwent partial menisectomy, normal cartilage

Group B:  patients who were middle aged (50-60) underwent partial menisectomy, nearly normal cartilage.

Group C:  patients who underwent TKA (total knee arthroplasty) end stage cartilage disease.

Tissue was obtained from the supra patellar fat pad

Investigated was:  colony forming efficiency; in vitro expandability;  cell proliferation rates; flow cytometry; in vitro chondrogenic differentiation;  immunohistochemistry; Polymerase chain reaction (PCR)

Results:  Nucleated cell numbers were greatest in the young group–although the significance was not listed.  No difference between the older groups.  No difference between all groups as far as cell surface protein expression. Expansion-ability to expand was retained only in young group-there were significant differences between all groups stated, however this was not clear from the graphic.

Proliferation group A was the highest, but do not know if this was significant.  Chondrogenic differentiation, immunohistochemistry, and PCR was not different among the groups.

Conclusion–age differs significantly with the respect to expandability and proliferation of SDSCs.

Commentary:    This study confirms work previously published in osteogenic potential D’Ipollito-1999.  Taken the knowledge a step further from the work of Im, et. al, Chondrogenic potential in older individuals-2006.  This study outlines what all practitioners of biological treatments should aspire.

1)  Excellent characterization and recording of tissue/cells

2)  Determination and recording numbers of cells being delivered-flow cytometry

3)  Capabilities of cells

By no means has this single poster summarized all the key requirements for these types of treatments, but not a bad job in a 6’x4′ space.

Dr. William Murrell will be co hosting the Biologic Orthopaedic Society Social, during the American Academy of Orthopaedic Surgeons Annual Meeting in New Orleans, Louisiana, USA– at the W Hotel in New Orleans with BOS founder Dr. Allan Mishra.

Dr. Allan Mishra is a board certified orthopedic surgeon and sports medicine specialist.  He pioneered the use of platelet rich plasma for sports related injuries and disorders.  He has published his research in elite journals and presented at conferences worldwide.  He has treated all types of patients from weekend warriors to collegiate, professional and Olympic athletes.

Dr. Mishra’s  research has been featured on the front page of the New York Times.  It has also been broadcast on Fox News, the CBS Evening News, and ABC News.

Dr. William Murrell together with Mr. Sumedh Talwalkar moderated the session for Upper Limb on Day 2 (March 7, 2014)  of the International Advanced Orthopaedic Congress held at the Mohammed Bin Rashid Academic Medical Center, Dubai Healthcare City (DHCC) Dubai, UAE. In this session Dr. Bill discussed treatment options of Clavicle Fractures in 2014. Dr. Bill was also joined by other distinguished speakers starting with Dr. Louay Jabban who gave a discussion on Complication After Later jet Surgery followed by Mr. Sumedh Talwalkar’s discussion on Total Elbow Replacement in RA and Hemiarthroplasty of the Elbow for Trauma. The session ended with an interactive question and answer portion.

The last day focused on Foot and Ankle, led by Professor Alvin Crawford and Professor Shah Alam Khan and followed by lectures given by Dr. Ayman Tardos Senior Specialist, Orthopaedic, Mafraq Hospital, Abu Dhabi, UAE, Dr. Ihab Marwan Abbas Orthopaedic Specialist, Al Ain Hospital, Al Ain, UAE, Professor Nicola Maffulli FFSEM Professor of Sports and Exercise Medicine Consultant Trauma and Orthopaedic Surgeon, Queen Mary University of London Centre for Sports and Exercise Medicine, Mile End Hospital, London, UK, Dr. Ravi Mittal Professor All  Indian Institute of Medical Sciences, New Delhi, India,  Dr. Shital Parikh Orthopaedic Surgeon(Sports Medicine), Cincinnati Children’s Hospital Medical Center, Cincinnati, Ohio, USA.

Session on Minimally Invasive Foot and Ankle led by Professor Sandro Giannini and Dr. Ravi Mittal, followed by lectures given by Professor Nicola Maffulli, Professor Sandro Giannini, Dr. Lorenzo Andaloro Consultant, Foot Clinic of Florence, Florence, Italy. This was followed with General Session with talks given by Dr. Neil Friedman , MB ChB, Center for Pediatric Neurology, Cleveland Clinic, Cleveland, Ohio, USA, and Dr. Edward Glaser Independent Researcher, Lyles, Tennessee, USA.

Afternoon session on Pediatric and Adolescent Foot Reconstruction were led by Professor Alvin Crawford and Professor Shah Alam Khan, with lectures given by Dr. Zaid Al Aubaidi Paediatric Orthopaedic Surgeon, Sheikh Khalifa Medical City, Abu Dhabi, UAE and Dr. Edward Glaser Independent Researcher, Lyles, Tennessee, USA.

Final sessions on the Adult Foot and Ankle Reconstruction led by Dr. Nicola Maffulli together with Dr. Edward Glaser, with lectures given by Professor Sandro Giannini and Dr. Nicola Maffulli.

Again all sessions were followed with robust Q & A, and just the right amount of controversial topics.

The last day of the conference ended with closing by Dr. William D. Murrell to bring Arab Health to and end.  Special thanks to the Cleveland Clinic for accrediting this program, and Bauerfeind-OTB, Amico, and Pharmalink for direct support of the Orthopaedic Program.

Dr. William D. Murrell, Chairman of the 10th Middle-East Orthopaedic Conference opened  Day 2 of the Arab Health Conference.  The session started with a topic on Pediatric and Adolescent Hip which was moderated by Professor Alvin Crawford. To start off, Professor Shah Alam Khan did a lecture on Pediatric Hip Arthroscopy, followed by Professor Alvin Crawford’s lecture on CDH and management. Then Dr. Shital Parikh finished the session with a lecture on Management of the Adolescent Hip.

This was followed by a session on Minimally Invasive Hip led by Dr. Adam Anz from the Andrews Institute, Florida, USA who talked about Biologics in hip and a another topic on Femoral acetabular impingement, this was  followed by Professor Hatem Galal Said’s lecture on Hip Arthroscopy-The Setup, and Hip labral repair. The morning session was closed with a talk on Hip Arthroscopy Rehabilitation by Mr. Anuj Punnoose, a senior Physiotherapist and Clinical Specialist, Trauma and Orthopaedics, Cambridge, UK.

Technology and Innovation was the opening topic for the afternoon session, Moderated by Professor Alvin Crawford together with Dr Shah Alam Khan. Lecture on Leverage technology-it is your friend was given by Dr. Jonathan Schaffer, Managing Director, Cleveland Clinic, and Information Technology Division, Cleveland Clinic, Ohio, USA. Followed by Dr. J. Michael Henderson, Chief Quality Officer, Cleveland Clinic, Ohio, USA, giving a lecture on Quality and safety play a role in innovating new models of care.

Session on Hip and Pelvis Trauma was led by Dr. Hatem Galal Said together with Dr. Chris Whately, followed by lectures from Dr. Khaleed Fareed, Dr. Humeira Badsha, Professor Ravi Mittal, Dr. Madhat Khafajy, Dr. Usama Hassan, and Dr. Zenet Khired.

The final session was focused on Adult Hip Reconstruction with lectures given by Dr. Chris Whately, Dr. Anil Arora, and Professor Ayman Ebied.

And just like Day 1, All sessions were followed with robust Q & A, and just the right amount of controversial topics.

Dr. William D. Murrell, Chairman of The 10th Middle-East Orthopaedic Conference thanked all of the sponsors and delegates for attending.  The day started very strong with a great keynote address from none other but Professor Lennard Funk, Shoulder Doc UK, from Manchester.  He gave a great lecture on Shoulder Instability in Competitive Athletes.  Additional lectures from Dr. Nitin Jain-Harvard Shoulder Service, Mr. Ian Bayley-BMI, London, Dr. Shital Parikh from Children’s Cincinnati finishing out the lectures on shoulder instability.

This was followed by a session on rotator cuff, led again by Dr. Nitin Jain talking about the nonoperative care and evidence as well as rehabilitation, then Dr. Guido Marra, Chief Shoulder and Elbow surgery at Northwestern, Chicago, IL, who spoke about corticosteroids and rotator cuff, treatment of full thickness rotator cuff tears was discussed by Professor Lennard Funk.

A session on perioperative complications and treatment was lead by Dr. John Tetzlaff, followed by lectures by Dr. Guido Marra, and Professor Lennard Funk.

Elbow disorders were discussed by a distinguished panel of speakers including Dr. Roger van Riet from Belgium, Dr. Ravi Mittal from AIIMs in India, Dr. Paolo Zuccu from Milan, and Dr. Guido Marra.

The final session featured a fantastic lineup of speakers including Distinguished Professor Alvin Crawford, Crawford Spine Center, Children’s Hospital, Cincinnati, OH, USA, Dr. Jose Manuel Rojo Manaute, Dr. Lucia Heras-Garcia, and Professor Vijay Gawande.

All sessions were followed with robust Q & A, and just the right amount of controversial topics.

Dr. William Murrell visited Arthrogen in January, to the village of Rottach-Egerner,  Germany and visited Professor Ulrich Schneider, to see the GOLDIC (gold induced cytokines) procedure in action.  Hopefully in the future we will be able to utilize this cutting edge solution.

December 2013 Sports Medicine In Tennis

As an Association of Tennis Professional (ATP) Chief Tournament Physician at the Dubai Duty Free Tennis Championship, Dr. William Murrell has been invited to attend the ATP Physicians meeting in New York City at the Hospital for Special Surgery. He will also be attending a preconference on sports medicine in tennis as well.

2nd Emirates International Orthopedic Conference – November 2013

Emirates Orthopaedic Association is having their 2nd Emirates International Orthopedic Conference here in Dubai this year. There is a great lineup of international and regional faculty including Dr. William Murrell, who will speak on shoulder acromioplasty, clavicle fractures, and calcific tendonitis. Dr. Bill will also moderate a session on shoulder trauma. This is a meeting not to miss.

Dr. Murrell visits UK to meet Prof. Lennard Funk

A great way to fellowship and learn from colleague shoulder surgeons is to travel. This time we have gone to Manchester and Wigan, UK to visit one of the best shoulder surgeons around, Professor Lennard Funk, at the Wrightington Upper Limb Unit-NHS, Wigan and Manchester Sports Medicine Clinic, Bridgewater Hospital, Manchester, UK. As with all visits, learned many new tricks, and most of all made an even better friend. Just sad that I forgot to take a picture together!!
